This is a behavior plan I created for a kiddo who had severe behavior issues. This particular plan was created because the student had a very hard time controlling his anger, he frequently ran from the class, and struggled with completing work.
The plan is broken down into three chunks of the day. The student has four goals to work towards. The student has three opportunities to earn a reward during the day. This helps for severe behavior needs because they feel success throughout the day.

The first card was created for a student who had a hard time controlling his anger. This card has 4 goals for the child to work for in order to earn a prize from the prize box.
The second card is the exact same behavior plan, but the child earns a computer break instead of a prize. You can chance the prize or goals depending on what your students' needs are.
