This 1/2 page exit ticket / assessment can be used in 3rd through 5th grade to determine whether students can use the library catalog to find nonfiction/informational books, find them on the shelf, identify tools to find information (index & table of contents) and record a fact. I have short class periods and can't often have longer assessments. I use this at the end of a school year, doing a quick review of information access skills, then have the students conduct an information search.
