This is something I created specifically for a student who is emotionally disturbed whom I am working with this summer. It is simple and easy to use. This version is editable. I saw what the student's teacher used during the regular school year and I thought this was a simple, small form that would help the student immediately see how he is doing that day.
We give the student three choices of reinforcers but if he gets three "X" then he loses the "A"-reinforcer.

This is an activity designed for any age group. It is a timed worksheet with 21 questions, all of which are random. The first question instructs the students to "read all before beginning" and the last says, "do only number 1." Therefore the students should end with a blank page. Most students dive right in and realize their error on the last question by which time it is far too late to erase and correct. Who doesn't run out of classroom supplies?! I sent this letter home mid-year last year to families as an fyi of the supplies our class was low on.
