Occupational Therapy Screening Request Form
This is a form school-based occupational therapists can use for screening students. I like to give this form to all of my case managers as well as any general ed teacher who requests it; the person filling it out can check off their concerns like fine motor skills, emotional regulation, difficulty with transitions, etc. There's also a section for the OT to fill out their observations and recommendations after completing the screening. I find that using a daily task list for therapy is how I can best deliver much of the information needed for effective feedback.
A task list can include, what tasks we are going to be doing, and what the learning targets are. The student knows what is expected and how they are doing at meeting their goals. We use picture schedules with pre-readers and all the benefits of picture schedules plus more are realized with task lists!
