Instrument Rental Contract

I'm always looking for an easy-to-use form that will help me keep track of student auditions. I have created something that categorizes student by name, class and/or school. It uses the 4 "Ps" Preparedness, Presence, Projection, and Pitch. There are also columns for "part requested" and "part given." I have a scale of 5 - 0 for help in assessing each child.
