I made this up for my class and thought it might help others. It's as a spreadsheet so you can tweak it according to your needs. (I had the students use a dictionary for some definitions and the internet for some so that they could practice various ways of looking up definitions. Yes, the answer sections are small, but my students needed practice writing in small spaces and controlling their penmanship. Feel free to tweak how you need.)
