Upper Elementary Library Skills Pre-test
This pre-test covers basic library and research knowledge. It includes the title page of EB White's 'Charlotte's Web' for students to identify elements of a title page. This is considered fair use of this single page. I have used it as both pre-test and post-test, and the title page can be changed to fit whatever needs you have.
