It uses the 4 "Ps" Preparedness, Presence, Projection, and Pitch. There are also columns for "part requested" and "part given." I have a scale of 5 - 0 for help in assessing each child.

I wanted incentive charts for grades 1-3 that didn't cost an arm & a leg! (I have aprox 300+ in grades 1-3!) so, I made my own, music themed, of course! Print out and copy as many times as you need, 4 per page, so save paper as well. I keep them on my cart in a square container clipped together with an index card with teacher's name on each group.
