Weekly Check-In Form
The Weekly Check-In is a comprehensive form used to check in on how students are doing. The questions ask about how they are handling their workload and how they are doing in and out of class (providing a baseline for SEL), and have the students perform a self-assessment to help them chart their current performance and chart their individual path forward. Careful consideration has gone into determining which questions are required versus which are optional to encourage maximum participation.
