This is a great fill in the blank State Report Template which all students can use. This is definitely a useful resource for English Language Learners and for students who may need a bit more guidance or modified work. The State Report template has sentence frames which include "labeled" blanks which specify what needs to be written on each line. I personally used it when I taught 5th grade, and it made a huge difference for my students.
