I love the idea of having a little something for the students and parents to take home with them during Open House/Meet the Teacher nights. I made this for myself and wanted to share it with you all in an effort to save you a little time. :-)
This is the perfect size for Gourmet brand microwave popcorn (Wal-mart). It was the cheapest and had the largest amount, so that is what I purchased for my classroom.
It is editable so that you can switch it around and add your name/subect.

ICEBREAKER!
Students each fill out a "Guess who I am" worksheet. Collect worksheets and redistribute to the class. Each student will then try and locate the student who filled out the paper they have in their hand.

Trip- Welcome Back labels
*Editable* I have three rotations of classes,and every year sixth grade has a week long trip to a science camp. My class is rewarded with their behavior with this label attached to m and m's candies (4-5).
