Hi this is a great way to screen your students for the year. I try to screen about 10-15 students/day and use the worksheet for documentation. There's a notes section at the bottom for absences or any notations that are needed (failed previous year, has new glasses, contact parent). I save each sheet in a binder and it keeps me organized.
