SLP Therapy Documentation Logs for Medicaid Billing FREE
I use these logs to keep track of my Medicaid minutes. I write in the number of minutes or simply check the days therapy was provided on the first log. On the second log, I typically circle or highlight the dates when therapy was provided. I also indicate in the area for amount and frequency of therapy if a student is seen in an individual or group setting as they are coded differently. I hope you find this resource useful.
