I use this assessment in my guided reading binder. I write the children's names down the side and keep track of their phonics progress throughout the year by giving them a checklist when I can see evidence of their mastery in guided reading time, in their writing, or on weekly spelling tests. It is a great tool to keep you accountable in making sure your kiddos get a strong foundation of the basics. I'd be lost without it!
