Used in my middle school (7t grade) classroom as a behavior consequence system. Students receive the paper on their desk as a visual warning, with behaviors checked. If the behavior stops, we have a conversation at the end of class and the redirection is recycled. If the behavior persists, we have a conversation at the end of class. Student and teacher sign the paper and it is sent home (via email and paper copy) for parent to see.
