I had my school print out a poster and laminate this for my classroom. If your school does not have a way to print in poster form, you can use your local Office Depot. You can also print this and put it in a sheet protector and hang on your bulletin board. I printed small ones and stuck them in a clear label pocket on each student's desk for a reminder to annotate while actively reading.
