For those using any of our RTI products (guide, screeners, goals, additional progress monitoring), this is a real-life example of a class tracking sheet that was used to document one fourth-grade class. All I did was delete the names of the students but left the scores/dates/etc. to give you an idea of how we used it. I left it in Excel so that you can easily adapt, delete, and edit it to match any assessments you are given, change dates, etc.
