I have been using this form for the past few years and find it VERY helpful at the beginning of year when trying to put together a (seems like "ever-changing") schedule. Easy to type in students (I usually put if they are speech or language in parentheses right after their name, i.e., Johnny Appleseed - S or John Smith - L) and how many minutes they are seen individually or in a group.
