This is an Excel file that works great for any middle or high school class. I fill out one template for each class and print it weekly; you'll need to input your student's names, the week (i.e. January 1-5) and edit the behavior codes to your preferences (it's set for my middle school ag. classes, i.e. points get removed for being on the phone or computer, talking, sleeping, disturbing others, etc. and each week day has a column.
