When students break a rule in a classroom that enforces the CHAMPS system for classroom management, use these slips to help reteach behaviors and communicate with parents. Students fill out the slip with the CHAMPS expectations. Then cut the slip on the dotted line. The student takes home the CHAMPS part for parent communication. The teacher keeps the small portion in the child's file. The small portions can be used to collect behavior data on individual children, or can be tallied onto a classr

This helps me prioritize and triage as necessary and helps hall monitors, admin, and clerks route the student in the correct place.
