I staple this inside of a manilla file folder and each student has their own folder. (I let them decorate it however they want.) I use a colored marker or crayon to fill in the top info, coordinated to their book level color. When it fills up, I just staple a new sheet on top. Easy to keep a running record of their AR tests.
