Daily Behavior Checklist (editable)
This is something I created specifically for a student who is emotionally disturbed whom I am working with this summer. It is simple and easy to use. This version is editable. I saw what the student's teacher used during the regular school year and I thought this was a simple, small form that would help the student immediately see how he is doing that day.
We give the student three choices of reinforcers but if he gets three "X" then he loses the "A"-reinforcer.

I usually print multiple of the weekly log on neon paper and put on a clipboard. I put it on my desk for easy access. Also, you can incorporate it for students to use on their own to monitor their behaviors to know what they will receive on their conduct folders.
