Make students accountable for homework assignments. Use these sheets to create an assignment notebook that students record their daily homework assignments. If the idea of an assignment notebook is overwhelming for your students, use sheets on a weekly basis and have the students keep the sheet(s) in their HOME folder. This survey is based on Howard Gardner's theory of multiple intelligences. It measures which intelligences (learning styles) students are more likely to utilize/apply. This survey includes an inventory with seven of the learning styles, is 10 pages long, and has a visual/graph so that parents can sketch the learning styles scores and visualize them.

Ever have kids who rush through their work? Problem solved! These "writer's licenses" can be issued, when a child rushes, give them a speeding ticket! Too many tickets and they loose their license.

Math Assessment Data Tracking Sheet for Students
Students use this sheet to track their progress on various math assessments throughout the year. Included are spaces for I-Ready, quarterly benchmark exams, and unit tests. Encourage students to follow these guidelines to remain organized daily. Spend class time regularly to discuss how to review for a test. Consider printing on colored paper to emphasize importance. 24-pound weight paper folds nicely and allows for the document to stand on its own for easy reference.
