IEP/GIEP Case Manager feedback form for gen. ed teachers
Being a case manager of an IEP/GIEP student can be challenging when it comes to gathering feedback from the general education teachers. This form allows that step to be easy for the teachers. You send this form out, they check the boxes for where the student rates in various categories, and finally, there is a place for comments.
