What is Surgery Book and Medical Leave note to Students
Do you or someone in your class have to have surgery? Use this "What is Surgery" book to help ease their minds. The unknown can be very scary for little kids. This information card is a parent survey for the first day of school. It is a quick card that I would print out on card stock to supplement parent information cards that were kept in the office since they were often hard to access. Information includes contact numbers, siblings, student medial needs and academic needs.
