I originally designed these mini-assessments to use as EXIT TICKETS throughout the school year. Each "exit ticket" has 5 questions that relate to a specific topic, allowing me to assess how well the student understands that topic. Each mini-assessment is self-graded by Google Forms, so you don't even need to do the work of grading it.
