School counseling student needs assessment

This is a survey to assess the needs of students and give a data-driven approach to planning school counseling services for the school year. This survey is intended to be given to students in early elementary school as it is a paper pencil with visuals for young learners. It includes 6 questions to get an understanding of your students needs and feelings. After students complete it you have lots of great data at your finger tips with little time invested!
