We use this in our first grade class to help students understand the consequences of their actions. They "Stop and Reflect" in order to make a better choice the next time.
*I have recently updated the document to include a middle grade version for 3-5 with more grade appropriate wording and formatting!
After it is completed, fold in half to keep private. The student signs it, the teacher signs it, then it goes home for the parent to sign.

This quiz is a fun and funny way to get students to realize they need to pay attention to directions.
There is a list of numerous steps to complete, which can actually be fun for students. HOWEVER, the first step says to "read all directions before continuing" and the last step says "only do..." so most students mess this up and do all the steps as they go along. This is a great activity to introduce and practice the idea of wants versus needs. There are large pictures that you can print and use in either a large group or small group and try to sort them. This generates great discussions! Then there is a smaller version(2 pages) to print that you can have the kids do on their own (cut and glue and sort) to assess their knowledge.
