IEP Goals and Objective Data Collection Sheet!

stacy Kingery
This spreadsheet is setup for weekly data collection though you can change it to be whatever your needs are. I have looked for a data collection sheet for a long time that will meet my needs in the classroom but nothing has done it quite like this one. Its to the point, easy for para's to fill out, easy to review for both staff and parents and very easy for data collection if monitoring data points and you want to turn it into a graph. This also could be used for homeschool tracking goals and

Participation Grade Tracker - Editable and Automatically grades!

Have you ever wanted to give a participation score, but felt like it wasn't concrete enough? I used this tracker in my classroom this past year, and noticed a marked increase in scholar engagement. We heard more voices and had more robust discussions because scholars had a clear picture of how to assess their own participation. This tracker is based on my Participation / Discourse Grading Rubric, and automatically populates a grade as students' daily participation data is entered. Grades are

Monthly Phone Logs

At my school we are required to turn into a phone log at the end of each month. I created this excel spreadsheet with monthly tabs on the bottom. At the beginning of each month I write each student's name. Then I document one time I spoke to the parent and briefly write what we discussed. This has been helpful for me to track my communication with parents.

How do you grade an oral presentation? How can you pay attention and write notes about the presentation at the same time? This rubric does half the work for you - just circle the applicable items during the speech and spend a minute at the end of the presentation tallying up the points. Divide the total by the number of possible points and you have your percentage grade. Ample room is available at the bottom of the rubric for comments. This is a simple feedback form/rubric that you can send to teachers regarding a student's tone of voice in social situations. This is primarily used for students that are working on adjusting their tone of voice (avoiding sarcasm, negative tone of voice, etc.). A "sample" goal is included at the top (adjust according to individual student needs) for teachers to refer to when filling out the form. You can then use the rubric scores to measure progress. The form is editable and can be downloaded o
