Description
After lots of research, I created this thorough yearbook club application. I used this one for 5th - 8th grade students at my school, but it can easily be modified to fit your needs. It could really be used as-is for high school - just take out where it says 5th- 8th ;)
The application is 6 pages long and includes:
*Student application (fill in, circle, and short answer essay questions)
*Teacher recommendations (2 teachers)
*Parent letter
*Parent approval form
There are a few places where you would need to put your contact info, etc. but almost all of the work is done for you! It's a thorough application, but not too difficult for students to complete.
I hope you get some good use out of it!
