Transition Workbook for Deaf/Hard of Hearing Students

Working with college/career-bound transition-aged deaf/hard of hearing students? This workbook is the perfect way to organize all of your resources, guide your sessions, and start to compile resources and information for/with your student before they leave high school. Download also includes access to a digital version of the workbook that you can copy for each of your students in Google Drive if you prefer a paperless option.

Possible Session Objectives:

- I can describe my personal strengths, interests, and challenges

- I can share concise information about my hearing loss

- I can share how I identify as a deaf/hard of hearing person

- I can plot my audiogram and describe what it means and what kind of listening situations may be challenging for me.

- I can describe my amplification, battery size/charger type

- I can identify members of my care team at school and outside of school

- I can identify goals that are personal and related to college/career path

- I can identify things that help me be a well-rounded student, including extracurriculars, sports, clubs, work experience, electives/classes I'm interested, and what skills I possess

- I can take career aptitude and personality assessments to help me determine potential personality, career, and degree matches.

- I can identify potential future jobs, major/minor, or other paths (gap year, apprenticeship) I'd like to do after high school

- I can learn more about colleges/universities I'm interested in and go through the application process

- I can identify my Vocational Rehabilitation Counselor and their contact information

- I can name laws related to postsecondary education and the workplace and my rights/protections

- I can describe what disability services is and accommodations available at colleges I am interested in

- I can brainstorm questions I have about disability services

- I can describe the FAFSA and different types of financial aid

- I can research tuition/fees at colleges I am interested in

- I can go through the scholarship application process

- I can identify my salary expectations for a future career, steps I need to take to be successful in that career, and a job-related goal

- I can list potential workplace environment challenges and accommodations I may need

- I can describe things I will do to take care of my personal needs independently after high school.

- I can compile contact information for important past/present members of my care team (educational and clinical)
