Clearly outlines considerations and documents decisions about ongoing intervention, increased support, any need for meetings and/or follow up.
Students that do not meet expected growth with tier 1 intervention and core
instruction should be represented in this area.
Tier 2 Intervention Consideration
This meeting should be held to review student data with school team and parents. The meeting form guides the team through a review of all academic data and provides an outline that allows the team to discuss any extenuating factors that may contribute to learning difficulties. The team will also make a determination about the next steps for student support.

An Academic Support Plan Consideration
This meeting should be held with the school team and parents if a student continues to struggle. The meeting form guides the team through a review of all academic data and provides an outline that allows the team to discuss any extenuating factors that may contribute to learning difficulties. The team will also make a determination about the next steps for student support, need to evaluation and possible accommodations.
Academic Support Plan
This plan is for students that progress through all 3 tiers of intervention, may have had additional testing and evaluation and may be identified as needing specialized instruction and accommodations. This form allows the team to document how the student has meet qualifications, the services that are to be provided to the student, and if specific accommodations will be provided
Student Accommodation Plan
Outlines accommodations options for presentation, responding, setting, and scheduling and offers areas to add specific accommodation that are not listed. Only accommodations that are appropriate for the student should be checked.

This form provides opportunity for documentation of parental acceptance for refusal of services or accommodations and explanation of supports offered.
