Therapy Notes, Data Collection, & Attendance/Service Delivery Calendar

This document provides a template for service delivery notes and data collection for Occupational Therapy service delivery in a school setting. This is a Microsoft Excel document. Although this is set up for an OT, it can be edited to address areas for physical therapists and speech and language pathologists. The note template provides a checklist of areas addressed as well as a place for writing a brief narrative note. The columns labeled Goal 1,2,3,4 are for collecting data relating to those goals. To the right of the spreadsheet is a place to copy/paste or type in the full goal for each number for reference as you are collecting data. There are additional features that can be added to the spreadsheet such as conditional formatting to show when service delivery is met in the calendar or averaging the data input in the columns, however these features were not added to this template since mastery criteria and service delivery time can vary student to student. There are 9 entry points on each page. It should/can print out on one page even though the preview does not look that way. The calendar for service delivery is for the 2018-2019 school year and dates are for Monday through Friday.
