Description

How often do you take time to find out how your students experience your class? As a student yourself, didn’t you have teachers and classes where certain things drove you crazy, and other things just confused you? If you were like most students, you probably never said a word.

Student feedback is arguably the MOST important feedback you can get as a teacher. This simple form gives you a tool for gathering it. And along with a printable PDF, you will also receive the original PowerPoint I used to create the form, so you can edit it for your own specific needs AND a link to a Google Form you can use to conduct the survey digitally.

This is the ELEMENTARY version of the form. It is appropriate for grades K-6, although if you teach in grades 4, 5, and 6, you may find that our SECONDARY/COLLEGE version is more appropriate for your students – take a look at the preview and decide for yourself.
