Student-Teacher Observation Forms

I have had the pleasure to be a mentor teacher within my school with new teachers as well as for student teachers. I have found the following worksheets helpful not only for myself, but for the wonderful student teachers I have had. I have included the following:

· Teacher observation sheet: Student teachers can use this sheet while observing their mentor teacher as well as other teachers within the building.

· Student Teacher observation form: Myself and my co-teachers have used this to observe our student teacher when they are leading class. I have included two versions -use what's best for you!

· Evaluation/Reflection form: Student teachers can use this during meetings with teachers when discussing observations. This form also allows them to make “next steps”.
