As part of every evaluation, re-evaluation, assessment or screening, it is ALWAYS imperative to see what the students will share with you in terms of their own impressions of their speech language and social communication skills.
This Form is my Go-To when I want to give my students a quick and efficient self-rating assessment, when I want them to reflect on their own speech language and social communication skills, or when I want to see where they are at in terms of their feelings about their overall language abilities.
The 17 questions touch on areas such as articulation, fluency/stuttering, executive functioning, dyslexia, as well as the emotional aspect of communication (social skills, social anxiety- selective mutism, self confidence) and knowledge of foreign language/bilingualism.
Tip: At the beginning of the year, when I set up interactive notebooks with my students, they fill out this form and glue it to the first page of the notebook. This helps us reflect on their current skills and helps them come up with an idea of what else they would still want to work on.
