Save time by printing this form and having an audition sheet ready for each student audition. I created this audition sheet for my own use and have seen so many other teacher comments who ask about the audition process.

The form is intended for teacher use only and asks the student questions like: "What role are you most interested in?", "Would you consider a narrator or singing role?" "Would you consider a dancing role?", "Do you feel comfortable performing in front of people?" , etc. Note: (I shred all audition sheets after role assignments have been made)

The form then asks the teacher to rate the student audition from 1 to 5 in various categories and also has a "comments" section to make extra notes.

This form helped me tremendously year after year holding auditions for K - 3rd and 4th - 6th grade musicals but could also be used through high school!
