This is a great activity to get to know teens. I have done it with two of my teen therapy clients over the phone, I like that it has unique ways of asking them things about themselves a typical "ice-breaker" might not have.
We used this as our first day of school activity, and the it was a huge success. I love the detail in the questions and they helped me learn so much from my new students.

Description

As someone who works with students that experience[d] trauma and have very diverse backgrounds, I have added a few prompts that I believe will help an educator better understand their students' situation at home. It touches on feeling about school, number of people in the household, what strong beliefs they have, sleep, etc. SEL encourages young adults to communicate clearly and effectively, practice eye contact, and social-skills- this worksheet may also be used as a community building exercise, warm-up, do-now, or as a guide for paired interviews.

The design and questions are a more mature take on another self-identity & culture worksheet I had created in the past for younger grade levels. This worksheet asks the students to fill out the info-graphic based on personal reflection. Students learn best when they have a sense of connection with their teachers, here I tried to create a noninvasive questionnaire so students can share about themselves for my knowledge as their educator, as well as their own experience investigating themselves.
