Description
Here are three needs assessments that are super easy...using Google Forms! Once filled out, you get instant data to use for your counseling program! Use this data to decide what lessons to teach throughout the year and what small groups to facilitate. These assessments are also helpful in determining what kind of school-wide initiatives you want to have in place.
Needs Assessment includes:
- Staff survey
- Parent Survey
- Student Survey
There are three Google Forms located in a shared file on Google Drive. You will need access to a Google Drive account.
There are also the same three forms to print as well!
How to Access
You will be prompted to MAKE A COPY (blue button). Make sure your logged into your gmail account so you can make the copy to your drive. To access the assessments... go into your google drive and find your copy. Open your copy and begin making edits, if you choose. Then send yours to your stakeholders to fill out (Parents, Staff, Students). From your Google Drive, you can see the results of your assessments.
