We're so glad you asked. Look no further for your guide:

Subjective- How did your student present? What external factors could have contributed to performance? What activity are you using?

Objective- Lines to describe your goal (two in total) with 10 boxes for data collection- easily calculate an accurate percentage every time!

Assessment- Record the % accuracy *and the cueing type and frequency.* Cueing types are organized with check boxes for your convenience, and you can circle the frequency of your cueing. *NOTE: the cueing frequency corresponds with ASHA's frequencies to indicate how often cues were administered (e.g. less than 20% of the time, 20-49%, etc.)*
