Profile Card Template for Spec. Ed. Student Information

When filled out with your student's information, this product provides a snapshot of important student information, particular to special education students. The purpose in creating this template was to have all key information on one page, therefore serving as a fantastic and organized quick reference sheet for a student's current levels of performance (with an area for supporting data), important annual and re-evaluation dates, disability, and services. Prior to creating this profile card template, I found that I had tons of information on students, but they were all in different places. Developing a profile card on each student seemed like the smart thing to do. This is a great tool to pass off to the student's receiving teacher, or just to have next to you when writing those cumbersome IEP progress reports. I have left the product as Word document, so you are able to edit it to best fit the needs of your students. I hope you find this product as helpful as I did!
