Use this visual, picture -based (real photos) inventory survey tool to assess reader & non-reader special education students ages 13-22 (moderate to severe disabilities) prior to annual IEP meetings, so that you can thoughtfully write a student’s ITP, transition plan, transition assessment, and/or transition IEP goals.

This 18 page assessment was created after years of experience working with transition aged students and their families. We use this annually with the majority of our students, in conjunction with formal assessments during re-eval years.
