Phonemic / Phonological Awareness Inventory Test, Primary Grades

This is a primary grades assessment comprised of 30 oral response questions to assess Phonemic Awareness. I did not create this inventory, but modified on that my school currently uses. I created a chart so that I could track kids progress over time, instead of flipping back to their first quarter results, then to the second...This type of record form would be great for strugglers, RtI (Response to Intervention), or when trying to document for 504/IEP's with Special Education Students.

Our school does not send this document home to parents, as it would be easy to memorize the few questions on this test and would invalidate it. Instead, we send a note stating their score and what is mandatory for promotion (a 27/30 questions for my district).
