This questionnaire was created by a monolingual, English speaking SLP. It was professionally translated into traditional Mandarin Chinese. The questionnaire features yes/no questions and a rating scale, so as to reduce open ended questions and clearly convey responses of the child’s caregiver.

Ideally, a bilingual SLP should assess ENL (English as a new language) children. However, a bilingual therapist or a translator is not always readily available. This is a tool that can be utilized during the screening process or until a translator or bilingual SLP become available.
