Description
This Google Form will track all communication between teacher/parents/guardians. It will collect information such as who the child is, what is the name of the person you spoke with, what relationship does that person have to the child, date/time of day, and what type of communication was used.
This form has drop down boxes for the following categories...
- PHONE CALL
- LETTER HOME
- IN-PERSON MEETING
Based on which selection you choose the form will automatically re-direct you to different questions.
Once you select the type of communication used the form will ask you several questions, with all options have a spot to write a summary of the conversation. The last question of the form is called FOLLOW-UP and it gives you a place to record any action plans or "next steps" that must be taken based off of the conversation (i.e. a notebook will start going home daily to be signed).

Keeping track of parent/guardian communication can be a challenge. This Google Form will keep all communication between teacher/parents in one place! We need this information to be easily accessible and often we have post-it notes, sheets of papers, emails etc. all over the place because we are jotting down our communication ON THE GO! One of my favorite parts about this digital form is being able to keep track of all of the communication information in one place EASILY! I often complete this form right from my smartphone! It records who you spoke with, dates/times of the conversation, what was discussed, and more. This automatically recorded data is an amazing way to keep track of all this information. Being able to pull up the exact data from this Google Form from anyplace is a huge benefit for teachers!
Google Forms are a great tool to gather data and have automatically saved results in a nice organized way! Use these next few slides to understand how the Google Form is set up, and how to maximize its usage. This form is EDITABLE so you can also customize it to fit the needs of your students!
