PT/OT/Speech Schedule and Goal Tracking

This document includes:

- A weekly session schedule in 15-minute increments, beginning at 7:30 through 4:30 for Mon-Fri. Edit in the doc or print and write your schedule.

- A goal tracking page to make sure you stay on track with your client's goals. Simply add dates in the top row you wish to monitor progress/goals so you can check it off for each client/student when it is completed.

- Another goal page in which you include the client/student's name, their goal, and frequency, to help organize and keep track of goals. It helps having all of the goals in one place for easy referencing.

- A session planner, which includes spaces for 3 activities for Mon-Fri, so you can plan ahead and stay organized with your interventions/activities. It includes a 'skills targeted' section to help you stay on track with the purpose of the ac

Instructions: Download the document in .docx as a Word document! Must have Microsoft Word to get the correct formatting. Edit the document as desired. You can add in client names in the schedule, but must first merge cells (highlight the cells desired, and click 'merge cells') to form a 30-minute increment.
