Since we know all students do not learn at the same rate, I suggest differentiating the use of this tool. Use the Teacher Data Record to track each student and quickly differentiate the assessments by giving each student the level they are still working on.

Each week you will have the needed data to differentiate your instruction. Assign games and activities to students based on their needs. You can even differentiate homework for each child.

I added a second version, Form B, of each assessment for you to use as a practice page to prevent students from memorizing the order of the answers.

I have also included a Student Data Record so students can track their own mastery of the strategies. This is an important activity to encourage your students to take responsibility for their own learning.

At the bottom of each assessment is a place for student goal setting; students choose 3 facts to practice on their own.
