This math survey is a quick and easy way for you to get to know your students feeling and attitudes about math, and then use that data to support a positive math culture.
*Please note: this resource is intended for upper elementary. You may need to administer orally to your students, depending on the grade level OR you can assign the paper version or google form version for students to complete independently.

>Math Survey Option 1 (25-questions): all questions focused on getting to know your students' math identity and attitudes + how they like to learn best in math class. There's two options for the follow-up reflection questions at the end, and two header options ("Math Mood" or "Math Survey")
