Description
Use these forms to send home with your kiddos if they ask to replace a book that has been lost or damaged. Copy on bright paper so they stand out and will be noticed at home!
One set of forms includes a note at the bottom indicating that an exact copy of the book can be sent in instead of payment—this way, families can bargain hunt to find a lower cost for the book than what’s on the bill!
