Intake Interview Form

This is an intake interview form used for school counselors and/or school psychologists to gather necessary information on students before providing them with services. It is a zip file with two versions of the interview form, both files within the zip are PDF files (both files are 5 pages each). These have a lot of insightful questions, great for all age levels, but if you are conducting the interview on elementary age students, you may need to omit certain questions as not all questions apply to their age level.
